Alta Skilled Nursing and Rehabilitation Center is owned and operated by Revive Health Senior Care. Revive Health Senior Care is headquartered in Reno, Nevada, making Alta Skilled Nursing and Rehabilitation Center the only locally owned and operated Skilled Nursing Facility in Reno.


ZACH GRAY, Owner & Administrator

Zach has practiced as a Nursing Home Administrator and Assisted Living Administrator in California, Illinois, Maryland, Washington, D.C., and Nevada. In addition, he maintains his licenses in New York and Virginia. Throughout his career, he has demonstrated the ability to manage nursing homes at a 5 star rating; and all of the nursing homes he has managed in the last 5 years have had 5 star ratings on CMS Nursing Home Compare. This is Zach’s first building that he has also owned while acting as Administrator. Zach has a Bachelors of Science in Healthcare Administration from Georgetown University, and a Masters of Public Health with a concentration in Policy and Management from Columbia University.


Chelsey Gray, VP of OPerations

Chelsey began her career in public service, working for the Mayor’s office in San Diego. She has an undergraduate degree in Sociology from the University of California, San Diego and her Masters Degree in Public Policy from George Mason University. After graduate school, Chelsey took a job with a large Government Contracting Firm, focusing on large data sets and the way they impacted public policy decisions. Chelsey spent years working on solving complex problems for the government and private businesses. After many discussions with Zach, it became clear that many of the problems facing Skilled Nursing Facilities are not being handled in an analytical or sophisticated manner.She’s partnered with Zach to transform the industry.


Kim Lee, RN, BSN, DON

Kim is the Director of Nursing at Alta. She has served in this role across a few organizations for the past five years, joining Alta in 2018. Kim has earned two bachelor’s degrees, one in Management and Organizational Development from George Fox University and one in Nursing from Linfield College-Nursing & Health Sciences. Kim has a dynamic leadership role, where she focuses on maximizing performance while providing evidenced-based clinical and operational oversight. She identifies opportunities to strengthen internal processes while improving safety, quality care and patient satisfaction. Kim is a forward-thinking nurse, who is able to think critically in difficult situations, and lead a staff to follow structured assessments and provide a very high quality of care to our residents.
